MINIMUM VACCINE REQUIREMENTS FOR STATE OF TEXAS 2009-2010

3 & 4 year olds:

• 4 doses DTP/Dtap (4th dose after 18 months)

• 3 doses polio

• 1 dose Hib after 15 months (unless primary series and booster completed)

• 1 dose MMR (on or after 1st birthday)

• 3 doses hepatitis B

• 1 dose varicella (chicken pox) on or after 1st birthday OR

• parent signature stating month & year child had chicken pox

• 2 doses hepatitis A

• PCV7 series complete according to recommended schedule

Kindergarten - 5&6 year olds:

• 4 or 5 doses DTP/Dtap (one dose after 4th birthday)

• 3 or 4 doses polio (one dose after 4th birthday)

• 2 doses MMR (on or after 1st birthday)

• 3 doses hepatitis B

• 2 doses hepatitis A (1st dose after 1st birthday)

• 2 doses varicella (chicken pox) (1st dose after 1st birthday) OR

• parent signature stating month & year child had chicken pox

7 – 11 year olds up to 6th grade:

• 3 doses DTP/Dtap/Td (last one after 4th birthday)

• 3 doses polio (last one after 4th birthday)

• 1 MMR after 1st birthday

• 2nd dose measles

• 3 doses hepatitis B

• 1 dose varicella (chicken pox) on or after 1st birthday OR

• parent signature stating month & year child had chicken pox

12 years & older or 7th grade:

• 3 doses DTP/Dtap/Td (last one after 4th birthday)

• Tdap booster within 5 years

• 3 or 4 doses polio (last one after 4th birthday)

• 2 doses MMR after (1st after 1st birthday)

• 3 doses hepatitis B

• 2 doses varicella (chicken pox) (1st after 1st birthday) OR

• parent signature stating month & year child had chicken pox

• 1 dose MCV [meningitis]
